Release of Financial Information

    We would like to have our students sign a form giving us authorization to
    release financial information to parents, etc.

     

    Has anyone developed a form that they would be willing to share? We are
    looking for something to keep on file that gives us permission to talk
    parents, guardians, etc. Any help would be greatly appreciated.
