Booster or Membership fees on tuition bill

I've been asked to look at the possibility of including alumni
membership and athletic booster fees on our tuition bills. I have a few
questions about the process:

* What fees of this type do you include on your bill?
* How is that fee added to the bill?
* Is it refundable?
* What happens if the student does not pay the fee? Could not
paying the fee cause the student to be blocked from registration or
prevent the student from obtaining a transcript?
