Account Agreement and Disclosure Statement that we created ourselves. We
have the student and parents/guardians sign it.
We also request the SS# for all parties who sign. Two copies of the
agreement and a postage paid envelope are mailed with our new student
orientation packets. We currently have about an 80% return rate.
If Suzanne will send me her fax number, I would be happy to fax her a copy
of our document.
