Suzanne,

Wartburg College (4 year liberal arts college, 1,600 students) implemented
such a form effective June 1, 2000 (the start of our 00-01 school year).
Attached is a Word file which represents our Student Payment Agreement
(SPA). In the Spring of 2000, we had all the existing students sign the
form before they could register for classes for the 00-01 school year.
For new students, we included the SPA with a mailing in May 2000 requesting
that they sign and return the form. Over the summer, we followed up with
new students who had not signed the form. We also added the Title IV
Financial Aid statement to this document, so we didn't have to create a
separate form.

We haven't had to pursue collection on any of these signed accounts, but
I'm sure we will in the near future. Good luck with the process.
