bursar-l: Automation of Forms

Is there anyone out there who has been able to automate various student
forms? What we are looking to do is have our Title IV authorization
form, FERPA, Direct Deposit, and Transfer of Aid to the Rocket Card form
out on our Student Self Service. We would like to be able to have the
students complete the forms on line and then have that information
directly feed into our system. We are a Banner system.

Has anyone tried this - accomplished this - would like to accomplish
this and have ideas to bounce around?

Any information would be appreciated.
